Reynolda campus undergraduate students seeking CES for a medical or mental health condition directly impacting their ability to engage academically must provide documentation of that condition from a qualified health provider.  Documentation should be submitted to medicalrequests@wfu.edu no later than three business days before the OAA submission deadline. This will allow adequate time for review and communication with OAA on your behalf. Graduate and professional students should contact their departmental student affairs officer:

Wake Forest School of Business

Matt Imboden, Chief Student Services Officer

Wake Forest School of Law

Branden Nicholson, Assistant Dean for Student Affairs

Graduate School of Arts and Sciences

Jennifer Rogers, Associate Dean for Students

Wake Forest School of Divinity

Brittani Hunt, Assistant Dean for Admissions and Student Services

International students with F1 or J-1 visa status should contact the Center for Immigration Services and Support (ISS) at iss@wfu.edu or 336-758-3739 before petitioning for CES.

Student-athletes should contact Compliance before petitioning for CES.

Students on scholarships or significant financial aid should consult with Financial Aid when taking CES.

Return from medical or mental health CES

A student’s resumption of full- or part-time status following time away in Continuous Enrollment Status (CES) is contingent upon a statement from appropriate mental health or medical authorities that the condition has been sufficiently addressed so that appropriate academic and social engagement can be resumed on this CES Resumption Form. It should be understood that in order to resume full- or part-time status, there must be a demonstrated period of stability that supports readiness to return to their academic program of study as determined by the student’s provider(s). Each case is considered individually.

Petitions for return should be received no later than three business days before the submission deadline to OAA. This will allow adequate time for review and communication with OAA on your behalf.

Students who need to establish follow-up care in the Winston-Salem area or on campus should contact us to schedule an appointment with one of our case managers before returning to campus. These appointments can be scheduled via Zoom at medicalrequests@wfu.edu.
